I certify that the facts contained in this application are true and complete to the best of my knowledge and understand that any falsified statements on this application may be grounds for the termination of any and all contracts and/or agreements made with the company. I authorize an investigation of all statements contained herein. The references and previous work history contacts listed are authorized to give you any and all information concerning my relationship with them, and release the company from all liability or any damage that may result from the utilization of such information.
